Student Physical Exams
Pennsylvania public school code requires that students upon original entry, 6th grade and 11th grade, be given a comprehensive appraisal of their health. The physical is to be completed within four months prior to the start of the school year. In addition, section 1402(c) of school code requires the completion of medical questionnaires which are to become part of the student's health record.
This requirement may be completed by the student's primary care provider (MD, DO, CRNP, or PA) or by the school provider.
Dental Exams
The Pennsylvania School Health Law requires Dental Examinations for children in kindergarten, grade three (3), and grade seven (7). These grades are selected because they represent critical periods of growth and development in a child's life. It is important that the school have a record of your child's dental status. During the screening, the dentist views the child's mouth and makes recommendations to the school. Notices are sent home regarding the dentist's findings.
School Dental Exams will proudly be provided by Geisinger Dental Services.
Please use the following "Permission Slip" to indicate your dental preference (school based or private dental exam) and return the form to the nurse's office in your child's school.
Individuals choosing to utilize a private dental exam must have the selected dentist complete the following "Private Dentist Report of Dental Examination of a Pupil of School Age" and return the form to the nurse's office in your child's school.

School Immunizations & Vaccinations
Attendance in Pennsylvania schools requires students to receive certain age-appropriate vaccinations & immunizations; unless the child has a medical or religious/philosophical exemption. Students require vaccinations upon entrance into all grades, with additional vaccinations needed by the beginning of 7th grade and again for attendance in 12th grade.
